ANDCare provides Nurse Practitioner led palliative care home visits for people living with serious illness or approaching end of life. Our focus is on comfort, dignity, and quality of life, while supporting families and carers every step of the way.

Care is individualised and delivered in the familiarity of your own home. We work alongside your existing healthcare team so you feel supported, informed, and confident.

Palliative Care Services

ANDCare does not replace government-funded services or specialist palliative care teams within the Northern Sydney Local Health District. Instead, we provide additional support, helping to fill gaps in care when needed.

We understand that time in hospital is time away from home. ANDCare works closely with hospital teams to support timely discharge planning and smooth transitions back home.

For those already at home, our focus is on helping you stay there, anticipating needs, planning ahead, and connecting you with the right supports so care feels achievable and well supported.

With over 30 years’ experience across hospitals, specialist palliative care units, residential aged care facilities, and community services, ANDCare brings deep understanding of both the healthcare system and the lived experience of serious illness.

Clinical Assessment

Assessment of pain and other symptoms

Monitoring changes in physical function and condition

Collaboration with your GP, specialists, and existing care teams

Symptom Relief & Comfort

Individualised pain and symptom management plans

Medication review, prescribing, and administration where appropriate

Practical, non-pharmacological comfort measures

Planning & Care Coordination

Open, honest conversations

Development or review of a personalised palliative care plan

Advance Care Planning and goals-of-care discussions

Coordination with other health services

Referral to local palliative care services when needed

Ongoing clinical support
