ANDCare provides Nurse Practitioner In-home Palliative Care for people living with serious illness or approaching end of life. Our focus is on comfort, dignity, and quality of life, while supporting families and carers every step of the way.

Care is individualised and delivered in the familiarity of your home. We work alongside your existing healthcare team so you feel supported, informed, and confident.

The ANDCare Approach

ANDCare does not replace government-funded services and specialist palliative care teams within the Northern Sydney Local Health District. Instead, we provide additional support, helping fill gaps in care when needed.

With over 30 years’ experience across hospitals, specialist palliative care units, residential aged care facilities, and community services, ANDCare brings deep understanding of both the healthcare system and the lived experience of serious illness.

What a Palliative Care Home Visit May Include

  • Clinical Assessment

  • Symptom Relief & Comfort

  • Planning & Care Coordination

  • Emotional & Psychosocial Support

End of Life Care (When Appropriate)

  • Symptom management in the final stages of life

  • Medication prescribing and administration

  • Set-up and management of continuous syringe driver infusions

  • Assistance with personal hygiene, mouth, eye, and skin care

  • Support with decision-making focused on comfort and dignity

  • Calm, compassionate presence for loved ones

Services are provided as clinically indicated. Not all supports are required for every person.
