Advance Care Planning Services (60-90mins)

Involves a dedicated one-on-one consultation to explore what matters most to you, your values, your preferences and your goals for future care. Ensuring your choices are known, honoured and central to care decisions especially at the end of life. Once your Advance Care Directive is completed, with your consent, it can be shared with your GP, your My Health Record, local hospitals and other health professionals involved in your care.

We provide you with resources, the original hard copy of your Advance Care Directive and a soft copy is emailed to you.

If attending a post workshop clinic the consultation is bulk billed.

If requesting a home visit:

  • Weekdays (8:00am–5:00pm): $250

  • Evenings: $300

  • Saturday: $300

Medicare eligible clients receive the rebate of $114.20 immediately on payment.

Nurse Practitioner Medicare rebates are based on the length of the visit. For home visits lasting 60 minutes or more, Medicare eligible clients receive the rebate of $114.20. This is the maximum rebate currently available for nurse practitioner home visits.

Private Palliative Care Services

Funding Options

Some services may be funded through:

  • Home Care Packages (HCP)

  • National Disability Insurance Scheme (NDIS)

Funding approval depends on individual circumstances and available funds. Any portion not covered will be privately billed.

  • Private Health Funds

In Australia, private health fund cover for in-home palliative care services delivered by an independent nurse practitioner is very limited. Clients are recommended to seek advice directly from their private health fund to determine if rebates may apply. Any portion not covered will be privately billed.

Travel Area

ANDCare provides home visits across Sydney’s Northern Beaches and the Lower North Shore.

Travel fees may apply outside this area and will be discussed in advance.

While urgent requests are responded to as quickly as possible, immediate attendance cannot always be guaranteed. We will always communicate openly and honestly with you. Service provision is at the soul discretion of the clinician and ANDCare reserves the right to decline or defer any referral or request without obligation.
