Advance Care Planning Services (60-90mins home visit)

  • Weekdays (8:00am–5:00pm): $250

  • Evenings: $300

  • Saturday: $300

Nurse Practitioner Medicare rebates are based on the length of the visit. For home visits lasting 60 minutes or more, eligible clients receive the Medicare rebate of $114.20. This is the maximum rebate currently available for nurse practitioner home visits.

Private Palliative Care Services (2 hour home visit)

  • Weekdays (8:00am–5:00pm): $500

  • Evenings (5:00pm–10:00pm): $600

  • Weekends & Public Holidays: $700

  • Overnight care (10:00pm–8:00am): $2,000

A $100 non-refundable booking fee is required to secure your appointment, which will be deducted from your initial consultation cost.

Funding Options

Some services may be funded through:

  • Home Care Packages (HCP)

  • National Disability Insurance Scheme (NDIS)

Funding approval depends on individual circumstances and available funds. Any portion not covered will be privately billed.

In Australia, private health fund cover for in-home palliative care services delivered by an independent nurse practitioner is very limited. Clients are recommended to seek advice directly from their private health fund to determine if rebates may apply. Any portion not covered will be privately billed.

Travel Area

ANDCare provides home visits across Sydney’s Northern Beaches.

Travel fees may apply outside this area and will be discussed in advance.

While urgent requests are responded to as quickly as possible, immediate attendance cannot always be guaranteed. We will always communicate openly and honestly with you. Service provision is at the soul discretion of the clinician and ANDCare reserves the right to decline or defer any referral or request without obligation.

How It Works

1. Get in touch and contact us directly via this website, or you can be referred by a health professional.

2. Initial Conversation (Triage)

A short phone consultation to understand immediate needs and discuss how we can support you.

3. Care Planning

With your consent, and once the $100 booking fee is received, we gather relevant clinical information from your GP, specialists, pharmacy, or My Health Record to ensure coordinated care.

4. Visit & Ongoing Support

Your Nurse Practitioner will attend the initial home visit and discuss next steps.
