Advance Care Planning is a vital process that supports you in making decisions now about the health care you would want in the future, particularly if you become seriously unwell and unable to speak for yourself. It’s about identifying what matters most—your values, preferences, and goals and making sure these are understood and respected by your loved ones and health professionals.
This service includes a dedicated one on one consultation to explore what matters most to you, your values, and your goals for future care. It is encouraged that your loved ones or Enduring Guardians attend, if possible, but is not necessary.
We support you to complete your Advance Care Directive (ACD) and with your consent, share it with your GP, My Health Record, local hospitals, and other health professionals involved in your care.
We provide you with resources, the original hard copy of your ACD and a soft copy is emailed to you.
Medicare rebates apply for eligible consultations.
